We serve an average of 110 – 120 patients daily who grant us the privilege of caring for them in their homes or wherever home might be – a skilled nursing center, an assisted living facility, retirement community, or group home. Each patient has a primary nurse who acts as their case manager and coordinates their care. Our nurses typically visit each patient 1-3 times each week. During a visit, the nurse assesses a patient’s overall condition and makes adjustments accordingly. If medications need to be changed the nurse will call the physician’s office and make the necessary arrangements. The same is true when a patient needs medical equipment or supplies, such as a hospital bed or oxygen. The nurse can call our contracted supplier and the items are delivered to the home. For patient comfort and safety and caregiver peace of mind, a hospice nurse is always on call after hours, on weekends and holidays. Hospice aides are available to all patients who desire this service. Chaplains, social workers, and HRC-trained volunteers are also available for additional support for our patients and family members.
